The 38-year-old information technology worker from New Mexico had a prescription. Her pharmacy had the drugs in stock. And her covered all but $25 to $50 of the monthly cost. For Griffin, the hardest part of using the new drugs wasn’t access. It was finding out that the didn’t really work for her. “I have been on Wegovy for a year and a half and have only lost 13 pounds,” said Griffin, who watches her diet, drinks plenty of water and exercises regularly. “I’ve done everything right with no success. It’s discouraging.” In clinical trials, most participants taking Wegovy or Mounjaro to treat obesity lost an average of 15% to 22% of their body weight — up to 50 pounds or more in many cases. But roughly 10% to 15% of patients in those trials were “nonresponders” who lost less than 5% of their body weight. Now that millions of people have used the drugs, several obesity experts told The Associated Press that perhaps 20% of patients — as many as 1 in 5 — may not respond well to the medications. It’s a little-known consequence of the obesity drug boom, according to doctors who caution eager patients not to expect one-size-fits-all results. “It’s all about explaining that different people have different responses,” said Dr. Fatima Cody Stanford, an obesity expert at Massachusetts General Hospital The drugs are known as GLP-1 receptor agonists because they mimic a hormone in the body known as glucagon-like peptide 1. Genetics, hormones and variability in how the brain regulates energy can all influence weight — and a person’s response to the drugs, Stanford said. Medical conditions such as sleep apnea can prevent weight loss, as can certain common medications, such as antidepressants, steroids and contraceptives. “This is a disease that stems from the brain,” said Stanford. “The dysfunction may not be the same” from patient to patient. Despite such cautions, patients are often upset when they start getting the weekly injections but the numbers on the scale barely budge. “It can be devastating,” said Dr. Katherine Saunders, an obesity expert at Weill Cornell Medicine and co-founder of the obesity treatment company FlyteHealth. “With such high expectations, there’s so much room for disappointment.” That was the case for Griffin, who has battled obesity since childhood and hoped to shed 70 pounds using Wegovy. The drug helped reduce her appetite and lowered her risk of diabetes, but she saw little change in weight. “It’s an emotional roller coaster,” she said. “You want it to work like it does for everybody else.” The medications are along with eating behavior and lifestyle changes. It’s usually clear within weeks whether someone will respond to the drugs, said Dr. Jody Dushay, an endocrine specialist at Beth Israel Deaconess Medical Center. Weight loss typically begins right away and continues as the dosage increases. For some patients, that just doesn’t happen. For others, side effects such as nausea, vomiting and diarrhea force them to halt the medications, Dushay said. In such situations, patients who were counting on the new drugs to pare pounds may think they’re out of options. “I tell them: It’s not game over,” Dushay said. Trying a different version of the new class of drugs may help. Griffin, who didn’t respond well to Wegovy, has started using Zepbound, which targets an additional hormone pathway in the body. After three months of using the drug, she has lost 7 pounds. “I’m hoping it’s slow and steady,” she said. Other people respond well to older drugs, the experts said. Changing diet, exercise, sleep and stress habits can also have profound effects. I think he is going through a mid-life crisis in later life. Is there such a thing? M.N. What you are describing might be called a late-life crisis . Such a crisis is described by Richard Leider and David Shapiro in their book, “What Do You Want to Be When You Grow Old? The Path of Purposeful Aging” (2021, Berrett-Koehler Publishers, Inc.). Here are some signs the authors identify: experiencing dissatisfaction, a loss of identity, an expectations gap and the feeling that life has peaked. Some also may believe life is a downhill journey. It’s having a feeling of being irrelevant. And it’s not just a guy thing. Both men and women are likely to experience it equally. Several events can cause a crisis in later life. Among them is the death of a loved one, an illness, money problems or no longer being able to walk the usual number of miles or having difficulty with poses in a yoga class. It also could be just boredom, doing the same thing day after day. Then there is retirement , which also can be a cause. For many, the loss of the work role can leave a huge gap in the reason to get up in the morning. For many, work is more than just a paycheck . It can provide a sense of purpose, social connection, structure for the day and knowing someone or some entity expects something from us. In a sense, it’s knowing we are needed. But other causes can cause a crisis in addition to the loss of the work role. For example, being out of touch with current technology, feeling unemployable and invisible in social situations or the marketplace. How do individuals know if they are experiencing a late-life crisis? Here are several questions that can serve as a clue, suggested by Leider and Shapiro. One key to avoiding or managing a late-life crisis is to not go it alone. “Isolation is fatal,” write Lieder and Shapiro. This makes me want to share examples from a recent discussion, not necessarily of a crisis, but increased awareness of the question of “who am I now and who do I want to be?” This discussion took place at a meeting with a group of women who are not going it alone. The group was made up of retired career women who are part of Renewment , a small grassroots movement that supports and inspires career women from work to retirement and beyond. Most were not going through a crisis, yet raised questions that indicated there are issues that may not reach the crisis level, yet still are important. For example, a retired successful entrepreneur asked the question, “Am I doing as much as I am capable of? Do I want to have an impact on a small or large scale?” A recent widow said, “Being a widow is not where I wanted to be at this time in my life; it’s not my identity. I feel so fragmented exploring who I am.” Another added, “I continue to be so hard on myself; always feeling I should do more; I am busy but not happy.” These same women and others in the group shared tips on avoiding problems that could lead to a late-life crisis. A therapist has emerged as an artist in New York and another was deeply involved in the election. A retired professor is traveling to Cuba and a former teacher launched a foundation. They have engagements that reflect a passion and sense of purpose. That’s the topic for next week. A point to remember: No one gets through this life alone. As Leider and Shapiro warn against isolation, remember that finding friends, colleagues, counselors or family members to talk about what’s going on can help avoid a crisis. She had surgery to put in a stent that saved her life. Shortly after her 2012 heart attack, O'Donnell shared her experience on her blog. During her 2015 television standup special, she spoke about how the experience changed her life. The segment included a heart attack acronym the comedian coined: HEPPP (hot, exhausted, pain, pale, puke). O'Donnell's candidness about her heart attack helped spread awareness about how it can present differently in women. She's one of countless celebrities over the years who have opened up about their health conditions, including breast cancer, HIV, depression, heart disease and stroke. When celebrities reveal and discuss their health issues, the impact can be far-reaching. It not only helps to educate the public, but it also can reduce stigma and inspire others. After actor John Ritter died of an undiagnosed aortic dissection in 2003, his wife, actor Amy Yasbeck, started the Ritter Foundation to raise awareness about the condition and help others avoid a misdiagnosis. Anecdotally, Gall Myrick said, people ask their doctor more questions about health conditions and request medical screenings. Celebrities can have a big impact because people tend to have parasocial relationships with them, Gall Myrick said. These are one-sided relationships in which a person feels an emotional connection with another person, often a celebrity. People may feel as if they know the basketball player they've watched on the court for years, or the Hollywood actor they've followed, she said. They want to comfort them after a health disclosure. Social media has only increased this feeling of familiarity, as celebrities regularly share mundane – but fascinating – details of their daily lives, like what they eat for breakfast, their favorite socks, or the meditation they do before bed. "We spend a lifetime being exposed to celebrities through the media, and over time, you get to know these public figures," Gall Myrick said. "Some feel like friendships." A study published in the journal Science Communication in 2020 compared reactions to actor Tom Hanks, who had COVID-19 early in the pandemic, and an average person with COVID-19. Researchers found that participants identified more with Hanks when it came to estimating their own susceptibility to COVID-19. The participants also felt more emotional about the virus that causes COVID-19 when thinking about it in relation to Hanks versus an average person. When a celebrity reveals a health condition, it's a surprise that may feel personal, especially if they are well-liked and the health issue is dramatic and sudden. "We feel like we know them, and the emotional response is what can then push people out of their routine," Gall Myrick said. Noar said a celebrity health story is often a more interesting and powerful way to learn about a health condition than just the facts, which can feel overwhelming. People are drawn to the slew of media coverage that typically follows a celebrity disclosure, he said. "Some of these high-visibility public figures' stories are now woven into some of these illnesses," Noar said. For example, Angelina Jolie is often linked to the BRCA1 gene mutation after the actor shared she had a preventive double mastectomy because of her elevated breast cancer risk and had her ovaries and fallopian tubes removed because of her increased risk for ovarian cancer. "It's a narrative, a story that humanizes the condition in a way that very informational communication really doesn't," Noar said. "People remember it, and it can potentially be a touch point." After a disclosure, patients may bring up a celebrity's story during a doctor's appointment and connect it to their own care. Today's multiplatform digital culture only amplifies celebrity messages. "You're seeing everyday people react to these events, and that can have a ripple effect too," Gall Myrick said. "We know from research that seeing messages more than once can be impactful. Often it's not just one billboard or one commercial that impacts behavior; it's the drip drip drip over time." Still, there's a cautionary tale to be told around the impact of celebrity health news, especially if the celebrity has died. An unclear cause of death may lead to speculation. Gall Myrick said that guesswork could potentially end up hurting rather than helping if patients were to act on misinformation or a lack of information. "Maybe the death was atypical or it needs more context," she said. "That's where advocacy groups and public health organizations come in. They need to be prepared for announcements or disclosures about celebrity deaths, and to fill in some of those gaps." American Heart Association News covers heart and brain health.
